How Ayurveda Understands Low Back Pain
Kati Shoola, Gridhrasi & Vata Dosha
Ayurveda identifies chronic low back pain as Kati Shoola and sciatica as Gridhrasi — both classified as Vata-predominant disorders (Vataja Nanatmaja Vyadhi). Vata governs all movement and nerve conduction in the body; when it becomes aggravated through aging, cold exposure, irregular routine, prolonged sitting, or heavy lifting, it manifests in the lumbar region (Kati Pradesh) as dryness, stiffness, shooting pain, and reduced mobility.
At the tissue level, classical texts describe the Shleshmadhara Kala — the membrane between the vertebrae responsible for secreting lubricating Kapha — as becoming depleted when Vata dominates, leading to friction, degeneration, and pain during spinal movement.
Kati Basti directly counters this imbalance: the warm oil is Ushna (heating) and Snigdha (unctuous) — exactly opposite to the Rooksha (dry), Laghu (light), and Sheeta (cold) qualities of aggravated Vata. Its penetrating (Sookshma) nature carries these corrective qualities deep into the muscles, ligaments, and nerve roots of the lumbar spine, restoring lubrication and easing pain from the inside out.

How Kati Basti is Performed

You lie face-down comfortably on the treatment table with the lower back exposed and fully relaxed.
A ring-shaped dam, roughly 2–3 inches high, is carefully built from black gram (urad) flour dough around the lumbar region to form a leak-proof reservoir.
Lukewarm herbal oil — such as Dhanwantharam Taila, Mahanarayan Taila, or Ksheerabala Taila — is gently poured into the dam until it fully covers the affected area.
The oil is left to rest for 30–45 minutes, topped up with warm oil periodically to maintain a steady, therapeutic temperature as it penetrates the tissues.
Once the session is complete, the dam is dismantled and a gentle massage is performed to enhance absorption of the remaining oil into the muscles.
