Introduction :

Grade 1 fatty liver , also known as simple hepatic steatosis, is the earliest and mildest stage of non-alcoholic fatty liver disease (NAFLD).

At this stage, fat begins to accumulate within the liver cells, but there is little to no inflammation or liver damage. Many people with Grade 1 fatty liver are unaware of the condition due to its subtle or absent symptoms.

What Is Grade 1 Fatty Liver?

Fatty liver occurs when excess fat builds up in the liver. Normally, the liver contains a small amount of fat, but when fat accounts for more than 5-10% of the liver’s weight, it is considered a fatty liver. Grade 1 indicates the least severe form, where fat is present, but liver function is still largely preserved.

Symptoms of Grade 1 Fatty Liver :

In many cases, Grade 1 fatty liver is asymptomatic, especially in the early stages. However, some individuals may experience mild, nonspecific symptoms due to the liver’s altered metabolism and fat accumulation.

1. General Fatigue

One of the most common symptoms is persistent tiredness or low energy levels. This occurs as the liver’s ability to filter toxins and regulate energy production is slightly impaired.

2. Discomfort in the Upper Right Abdomen

While not a symptom felt by the patient, blood tests may show a mild increase in liver enzymes such as ALT (alanine aminotransferase) and AST (aspartate aminotransferase), indicating liver cell stress or injury.


Who Is at Risk?

Certain individuals are more prone to developing Grade 1 fatty liver:

  • People with obesity or overweight
  • Those with type 2 diabetes or insulin resistance
  • Individuals with high cholesterol or triglycerides
  • Those who consume a high-fat, high-sugar diet
  • People with sedentary lifestyles
  • Individuals with polycystic ovary syndrome (PCOS) or metabolic syndrome

Diagnosis :

Since symptoms are often vague or absent, Grade 1 fatty liver is typically discovered during:

  • Routine ultrasound imaging
  • Health check-ups for elevated liver enzymes
  • Fatty liver screening in high-risk individuals

Ultrasound is the most common, non-invasive method to detect fatty changes in the liver.
