What is Uttarbasti Treatment?
Uttarbasti (also called Uttaravasti or Uttar Basti) is a specialized Panchakarma therapy in Ayurveda that involves instilling medicated oils or herbal liquids into the pelvic organs via the urinary or genital passages.
Literally, it is an "enema through the upper (Uttara) passage". This procedure is done under strict hygienic conditions by an experienced Ayurvedic practitioner. It aims to deliver healing herbs directly to the reproductive and urinary tracts.

Uttarbasti Procedure in Female
The female Uttarbasti procedure follows gynecological best practices combined with Ayurvedic steps, allowing the medicine to act directly on the uterus and tubes.
Initial Prep
The woman's bladder and bowels are emptied. A medicated oil abhyanga (massage) and warm steam to the lower abdomen are given to relax the pelvis.
Positioning
The patient lies on her back with knees bent (lithotomy). A routine per-vaginal exam is done to check uterus size and any pathology.
Cervical Access
A sterile speculum and retractor expose the cervix. The cervix is grasped with forceps, cleansed with antiseptic, and the external os is dilated.
Cannula Insertion
A soft intrauterine cannula is gently passed through the cervical canal up to the uterine fundus. The patient's body is tilted (reverse Trendelenburg).
Oil Instillation
Using a syringe, 10–15 ml of warm medicated oil or ghee is slowly injected through the cannula into the uterus. Drops are given until the prescribed amount is delivered.
Completion
The instruments are withdrawn. The patient rests, and blood pressure/pulse are monitored for 1–2 hours. Mild cramping may occur but usually resolves quickly.

Is Uttarbasti Painful?
Uttarbasti is generally not painful. Since no major surgery or cutting is involved, patients usually feel only mild sensations. The insertion of the catheter or cannula is done with lubricant and care, so it is comparable to standard medical catheterization.
Many report just slight pressure or fullness, not sharp pain. The medicated oils (which are often warm and soothing) can cause a sensation of relief rather than discomfort. Any cramping or irritation afterward is typically mild and short-lived.
